Calon Penerima Beasiswa Prestasi Mahasiswa Tahun 2020

Ketentuan sebagai calon penerima beasiswa

Selanjutnya, kepada seluruh Calon Penerima Beasiswa Prestasi Mahasiswa memperhatikan beberapa ketentuan berikut ini :

  1. Calon penerima Beasiswa Prestasi Mahasiswa yang dinyatakan LULUS, harus mengupload dokumen dibawah dalam 1 (satu) file format pdf berukuran maksimal 2 mb, paling lambat tanggal 30 Oktober 2020 melalui laman masing-masing pada Sistem Informasi Beasiswa (SIBEA)  (laman : https://kemahasiswaan.uin-malang.ac.id/beasiswa/login);
    1. Halaman depan buku rekening Bank Rakyat Indonesia (BRI) yang masih AKTIF atas nama mahasiswa calon penerima beasiswa;
    2. Pakta Integritas sudah bertandatangan calon penerima beasiswa, UNDUH FILE;
    3. Kwitansi sudah bertandatangan calon penerima beasiswa, UNDUH FILE;
    4. Surat Pernyataan Pertanggungjawaban Mutlak bertandatangan calon penerima beasiswa, UNDUH FILE.
  2. Calon penerima Beasiswa Prestasi Mahasiswa mengirimkan dokumen asli (sesuai pada point satu) melalui pos atau sejenisnya ke alamat :

Kantor Bagian Kemahasiswaan dan Alumni, Gedung Sport Center Lantai 01, Jalan Gajayana 50 Kota Malang.

  1. Calon penerima Beasiswa Prestasi Mahasiswa yang tidak mengirimkan dokumen asli (sesuai pada point satu) melalui pos atau sejenisnya, maka proses penyaluran beasiswa tidak dapat dilakukan;
  2. Penerima Beasiswa Prestasi Mahasiswa tahun 2020 harus membuat laporan pertanggunjawaban setelah menerima dana beasiswa,
    1. Scan Rekening koran atau melakukan cetak buku rekening yang menunjukkan dana Beasiswa Prestasi Mahasiswa telah masuk ke rekening masing-masing mahasiswa (halaman bukti masuk beasiswa di Buku Rekening);
    2. Scan bukti penggunaan dana beasiswa prestasi mahasiswa seperti slip pembayaran UKT dan atau kwitansi/nota dari penggunaan dana beasiswa;
    3. Membuat tulisan berbentuk makalah yang berkenaan dengan Organisasi Kemahasiswaan yang dipimpin (khusus bagi ketua Ormawa),

      Kisi - kisi penulisan sebagaimana berikut ;

          • Visi misi organisasi;
          • Program kegiatan;
          • Evaluasi kegiatan (peluang dan tantangan);
          • Laporan perkembangan jumlah anggota selama 3 tahun terakhir (evaluasi rekrutmen yaitu sosialisasi, metode perekrutan, diklat);
          • Harapan kelangsungan organisasi kedepan berkaitan dengan penyiapan SDM berkarakter dan unggul.
    4. Semua dokumen diupload pada laman masing-masing pada Sistem Informasi Beasiswa (SIBEA);
    5. Laporan pertanggungjawabaan MAKSIMAL SATU BULAN SETELAH PENERIMAAN DANA BEASISWA;
    6. Penerima bantuan Beasiswa yang tidak membuat laporan pertanggungjawaban, maka akan dikenakan sanksi dengan tidak diperkenankannya mengikuti program Beasiswa pada tahun berikutnya.
